		Sample Submission Confidentiality S & J LAB maintains strict confidentiality with its clients unless otherwise waived by written permission of the client. All work is coordinated under the strictest internal security to assure complete confidentiality. Financial Information Terms are net 30 days payable upon reporting. A surcharge is added for RUSH services. A finance charge of 1.5% per month may be added to overdue balances. Sample Submission Submitted samples should be accompanied by an Analysis Request Form or with a list of analytical instruction on your company's letterhead with purchase order.           Please provide the following information:                       - Company name and billing address                     - Name, phone number & e-mail address of contact person                     - Names, address or e-mail address of where the analytical reports should                       be sent                     - Description of sample (sample type, source, time and date of collection)                     - The full name of analysis                     - Estimated concentration levels (if known)                     - The date the results are needed           Please provide adequate amounts of samples for analysis. Samples must be shipped in a way that will not destroy or alter the identity of measurable components. Pack perishable products in ice packs or dry ice and ship via overnight express carrier for next-morning delivery (for perish foods). Samples should be delivered during regular business hours (Monday through Friday, 9:00am - 5:00pm EST). Weekend deliveries can be arranged upon request.           Please note that S & J Laboratories, Inc. reserves the right to refuse to analyze any sample submitted. Sample Retention Submitted sample will be retained for 30 days following the reporting of the results. Extended sample retention can be arranged and additional charges may be incurred to the customer. Recheck Analyses Rechecks performed at your request may be charged to you based on a comparison of analytical results. Turn-Around-Time Turn-around-time varies for different analyses. Typical turn-around-time is seven working days. RUSH analysis can be performed. Please contact us with details about your requirements for a more precise turn-around-time. Report and Invoice A written report will be sent by mail to the contact person. Results can be faxed or e-mailed if requested. Invoice will be issued to the accounts payable with a 30-day term.
